Based on observation, the licensee did not comply with the section cited above: fire extinguisher which was last serviced in April 2021, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will get a new fire exitinguisher or get the current fire extinguisher serviced by Plan of Crrection dude date, July 8, 2025.

Based on observation, the licensee did not comply with the section cited above: fire and disaster drill log which was last completed on 03/04/2024, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a drill and submit proof by Plan of Correction due date, July 8, 2025.

Based on observation, the licensee did not comply with the section cited above: LPA did not observe a life ring with a minimum exterior diameter of 17 inches and labeled as approved by the United States Coast Guard,which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed razor during visit. Deficiency cleared today.

Licensee last completed it on 09/11/2020 which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. Child 3, 4, 5, 7, 9 and 10 do not have immunization records on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit immunization records for child 3, 4, 5, 7, 9 and 10 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 2 and 6 immunization records need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview, the licensee did not comply with the section cited above. Licensee did not have a Safe Sleep log for infants in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a Safe Sleep log for infants for the week of 03/06/2023-03/10/2023 to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. Licensee's daughter left at about 1:10pm to pick up school age children leaving licensee with nine day care children. Licensee was over capacity which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement how she will remain in capacity to CCLD by POC date.
