Based on record review, the licensee did not comply with the section cited above. Assistant Jenna Jimenez does not have a current TB test on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a current TB test for assistant Jenna Jimenez to CCLD by POC date.
Based on observation, the licensee did not comply with the section cited above. Licensee's infant grandchild was asleep on the second floor which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will subit a statement stating she understands children must be on the same floor as licensee while sleeping. She will state what changes she has made to ensure this violation is corrected.
Based on interview, the licensee did not comply with the section cited above. Licensee does not have a sleep log for infants which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will begin to maintain a sleep log for children and submit proof to LPA by 11/10/2021.
