Based on interview and record review, the licensee did not comply with the section cited above, Safe Sleep log was not completed today, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and adult daughter understands that safe sleep log must be made everytime an infant sleeps while in care.

Based on observation, the licensee did not comply with the section cited above, LPAs observed bucket filled with about 5 inches of water in the backyard, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee emptied water during today's visit. Deficiency cleared today.

Based on observation, the licensee did not comply with the section cited above, LPAs observed dog feces in the back right playground area. Licensee’s daughter stated the last time the kids played outside was yesterday (2/18/2025), which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Adult daughter cleaned up the feces. Deficiency cleared today.

Based on observation and interview, the licensee did not comply with the section cited above. The carbon monoxide detector was not working. Licensee stated she turned it off the other day because it was beeping and turned it back on during the visit which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee turned detector back on during visit and it worked. Deficiency cleared today.

Resolution: Licensee and her family cleaned the kitchen area including the floors during visit. Deficiency cleared today.

Based on observation, the licensee did not comply with the section cited above. LPA observed the area under the swings is frayed and is a tripping hazard for children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a photo frayed material being replaced or removed for safety of children.

Resolution: Licensee removed her son during visit and understands infant shall sleep in a crib or play pen on a flat surface. Licensee was given website regarding safe sleep and shall read regulations and send LPA an email stating she understands Safe Sleep Regulations.
