Based on interview and record review, the licensee did not comply with the section cited above. LPA observed licensee's son Enrique Martinez has not obtained criminal record clearance which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee states son has an appoitment today and will ensure he is fingerprinted and cleared.
Based on observation, the licensee did not comply with the section cited above. LPA observed dog feces in the backyard play area for children which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee's daughter picked up feces during visit.
Based on interview, the licensee did not comply with the section cited above. Licensee's daughter and son in law moved back into the home approximately 6 months ago. Licensee did not notify the Department which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee completed a new LIC279 and LIC279B with updated adults and children who live in the home during visit.

Resolution: Child's mother came during visit to show updated immunizations for child. Mother was not able to print record but will get an updated record at child's next wellness check.
Based on interview and record review, the licensee did not comply with the section cited above. Licensee's son in law Jose Sanchez does not have a current TB test on file. which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a current TB test for son in law by 12/22/2021.
