Based on observation and record review, the licensee did not comply with the section cited above by not completing lead testing by January 1, 2023 which poses a potential health, safety or personal rights risk to persons in care.
Based on record review, LPAs identified that the incorrect live scan form was used for one of the teachers. This resulted in incomplete criminal record clearance. Therefore, licensee did not comply with the section cited above in one out of three persons which poses an immediate health, safety or personal rights risk to persons in care. This is an amended report. The type A violation has been changed to a type B violation.
