Based on observation, the licensee did not comply with the section cited above. LPA observed an infant sleeping in a play yard with a blanket, bib and bottle which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee was made aware of the immediate health and safety risk of loose articles and objects in cribs and play yards. Licensee removed all objects from play yard during time of visit. Licensee was provided PIN 20-24 Safe Sleep at time of visit.
Based on observation, the licensee did not comply with the section cited above. LPA observed knives in pull out drawer and cleaning compounds located in an unsecured lower kitchen cupboard accessable to children in care. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee is aware of this regulation and was informed of the potential health and safety risks of knives and cleaning compounds located in accessable and unsecured drawers and cabinets. Licensee relocated all items to a higher location within the kitchen during visit.
Based on observation, interview and record review, the licensee did not comply with the section cited above. LPA was unable to review fire/disater drill log at time of visit, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee understands this regulation and was provided a new drill log during visit. Licensee will conduct a fire/disaster drill and provide proof to LPA on or before the stated POC date of 08/18/2023
Based on observation, interview, record review, the licensee did not comply with the section cited above. Licensee did not have completed Mandated Reporter Certificate for review at time of visit which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee understands the importance of this regulation and the need to complete/renew Mandated Reporter Certificate every two years and have available for LPA to review. Licensee will complete/renew Mandated Reporter Certificate and provide copy to LPA on or before POC date of 08/18/2023.
Based on observation, interview, record review, the licensee did not comply with the section cited above. Licensee's CPR card expired 07/2023, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee understands the importance of this regulation and will enroll in renewal class/renew EMSA cpr training and provide LPA with proof of completion on of before the stated POC date of 08/18/2023.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 out of 1 infant child files reviewed. LPA did not observe completed LIC9227 in infant file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee understands the iomportance of this regulation and will obtain a completed LIC9227 for infants in care and show proof of completion to LPA on or before of stated POC date of 08/18/2023.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 out of 1 infant child files reviewed. LPA did not observe completed 15 minute sleep checks in infant file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee understands the importance of this regulation and will complete required 15 minute sleep logs for infants from this date 07/18/2023 and beyond and show proof of of one week of completed logs to LPA on or before of stated POC date of 08/18/2023.
