Based on record review, the licensee did not comply with the section cited above S1 Mandated Reporter Child Care Providers Training (AB1207) was completed on 12/11/2022 and there is not a current Mandated Reporter Child Care Providers Training (AB1207) on file for S2, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have staff 1 (S1) and staff 2 (S2) complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to provide proof of S1 and S2 Mandated Reporter Child Care Providers Training (AB1207) certificate of completion to Community Care Licensing by the end of the business day on the POC due date of 09/25/2025.

Based on record review, the licensee did not comply with the section cited above. Child 1 (C2) does not have an Infant Sleep Log that documents the time of each 15-minute check at the time of the visit. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document and maintain an Infant Sleep Log for C2 that documents the date, infant’s name, and time of each 15-minute check. Licensee agrees to provide proof of the 15-minute checks for C2, to Community Care Licensing by the end of the business day on the POC due date of 09/25/2025.
