Based on record review, the licensee did not comply with the section cited above, staff 1 (S1), staff 2 (S2), and staff 3 (S3) Mandated Reporter Child Care Providers Training (AB1207) was completed January 2023 and there is not a current Mandated Reporter Child Care Providers Training (AB1207) on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have staff 1 (S1), staff 2 (S2), and staff 3 (S3) complete Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to provide proof of S1, S2, and S3 Mandated Reporter Child Care Providers Training (AB1207) certificate of completion to Community Care Licensing by the end of the business day on the POC due date of 08/25/2025. www.mandatedreporterca.com

Resolution: Licensee agrees to obtain proof of S2’s required immunization of MMR and maintain proof at the facility. Licensee agrees to provide proof of S2’s MMR required immunization to Community Care Licensing by the end of the business day on the POC due date of 08/25/2025.
