Based on record review, the licensee did not comply with the section cited above as C1 and C4 were missing proof of the 15 minute sleep logs, the provider stated that she conducts 15 minute sleep checks but does not document them which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document and provide a copy of the 15 minute sleep logs for C1 and C4 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 07/24/2023.

Based on record review, the licensee did not comply with the section cited above as Licensee was unable to provide proof of the Mandated Reporter training for herself and her spouse, Licensee stated that she completed the training 3 months ago but was unable to locate the certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide proof of the Mandated Reporter Child Care Provider Training (AB1207) for herself and her spouse to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 07/24/2023.

Based on record review, the licensee did not comply with the section cited above as all children's records were missing proof of the LIC282 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have all parents of children enrolled fill out the LIC282 and agrees to place proof in children's files for future review. Licensee agrees to send proof of the LIC282s to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 07/24/2023.
