Based on observation, interview, and record review, the licensee did not comply with the section cited above in maintaining the documentation of the emergency earthquake/fire drills log which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review the licensee did not comply with the section cited in which the immunization records were kept in the staff files for review which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, the licensee did not comply with the section cited above in the stairs barricade or gate to the off limits area was not present during inspection and a three year old child was in care which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and interview the licensee did not comply with the section cited above. LPA observed mulitple items in the back yard that are are accessible to the children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove the items in the back yard that do not pertain to the child care and provide proof of the debris removal to the Department by the POC date of 6/16/2023.
