Based on observation and interview, the licensee did not comply with the section cited above in fireplace was not propertly screened, which posesa potential health, safety or personal rights risk to persons in care.
Resolution: LPA was informed by Licensee's spouse the fireplace cover had broke and needs to be replaced. Licensee will get a cover for the fireplace and send photo to LPA Flores via email by 5/28/24.

Resolution: Licensee's spouse will complete the Mandated Reporter course via online (AB1207) and send copy of Certificate to LPA Flores via email by 5/28/24.
