Based on observation and interview, the licensee did not comply with the section cited above in, the pool gate on the side yard was open at the time of inspection. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee was cited for an immediate threat. Licensee shut the gate upon request of LPA Flores during inspection. A Civil penalty will be issued and licensee will be required to come into an informal office conference prior to capacity increase is approved.
Based on LPA's observation, the licensee did not comply with the section cited above in,area by the bathroom has dirty laundry piled and there was a pair of womens underwear in the bathtub of the bathroom utilized by children, broken glass tile on the back patio, dog fecis in the back bedroom that is marked off limits, but was open and easily accessible to children in care. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will clean the bathroom area, remove toothbrush from drawer, have dirty laundry removed and placed in an area inaccessible to children, and keep son's bedroom locked during day care hours. Licensee will send photos to LPA Flores via email of all items corrected by 7/12/24.
Based on LPA's observation, the licensee did not comply with the section cited above in power tools were on the kitchen table upon entry of analyst's,there were knives in the kitchen sink, open bleach bottle under the kitchen sink, cabinet next to bathroom has paint, can, toxic products that were accessible to chidlren in care. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: licensee removed the power tools from the kitchen and placed in the garage, licensee will clean up back patio and keep backyard off limits until clean, keep cabinet under kitchen sink locked at all times, lock cabinet in hallway, and move paint can to the garage. Licensee will send photos of cabinets locked and back patio cleaned by 7/12/24.
Based on observation, the licensee did not comply with the section cited above in , the backbedroom was not locked or made inaccessible to children at time of inspection, there was dog fecis on the floor and the outlets were uncovered. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee cleaned up the dog fecis during inspection. Licensee will keep the room inaccessible to children with either a child safety door handle, or lock the room at all times during day care hours.
Based on interview and record review, the licensee did not comply with the section cited above in three out of 11 children's files were not available at the time of inspection, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee was advised on 5/21/24 that a child's file was needed for biological children and they need to be listed on the roster to be in compliance with Title 22 regulations. Licensee had immunization records for the three children. Licensee will create files for three biological children and send evidence to LPA Flores via email by 7/18/24
Based on record review, the licensee did not comply with the section cited above in, the roster was not updated with three children present at the time of inspection. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: licensee was advised on 5/21/24 to update roster with all information and to add biological children to the roster. Licensee will update roster and send proof to LPA Flores via email, by 7/18/24

Resolution: Licensee had her biological children picked up by relative during inspection. Licensee took in two more children as biological children were leaving, making the ratio at 8 to one. Licensee will be required to attend an informal conference at the Regional office prior to capacity increase approval.

Resolution: Licensee will obtain copies of immunizations/and or school enrollment form for the children enrolled in care by 6/04/24 and send to LPA Flores via email.
Based on record review, the licensee did not comply with the section cited above in four out of five children's records did not have LIc995A on file, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain LIc995A for each child enrolled and send proof to LPA Flores via email by 6/04/24.
