Based on observation the licensee did not comply with the section cited above where a life ring was not observed. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will purchase a life ring and provide proof of such to LPA by POC date.

Based on observation, the licensee did not comply with the section cited above in where the lock to the gate was measured at 58 inches. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will correct the fence, and move the gate lock up to 60 inches and provide proof of such to LPA by POC date.

Based on observation the licensee did not comply with the section cited above where the bottom of the gate measured greater than 2". This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to bring the bottom of the gate into compliance and provide proof of such to LPA by the POC date.

Based on observation the licensee did not comply with the section cited above where there was not a pool alarm. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will obtain a pool alarm and provide proof of such to LPA by POC date.

Based on record review, the licensee did not comply with the section cited above in child #1 does not have a sleep log on file at time if inspection. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA Reviewed the infant safe sleep plan and regulations with llicensee and her spouse. LPA gave licensee a copy of what an infant safe sleep log looks like and she will begin the process as of 5/26/23. Licensee will send photo of infant safe sleep log to LPA Flores via email or text by 5/26/23.

Based on record review, the licensee did not comply with the section cited above in child #1 does not have immunizations on file at time of inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: child #1 does not have immunizations on file at time of inspection; Licensee will obtain immunization records and/or school enrollment forms for children in care that are missing their forms

Based on record review the licensee did not comply with the section cited above in two out of six childrens files do not have the LIC995A on file at time of inspection, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain signed and completed forms from parents by 06/08/23 and send LPA Flores via email or text by 6/08/23

Based on record review, the licensee did not comply with the section cited above in three out of six children's files do not have the LIC700 Emergency information on file at time of inspection.This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain signed and completed forms from parents by 06/08/23 and send LPA Flores via email or text by 6/08/23

Based on records review, the licensee did not comply with the section cited above in that the Licensee nor her two assistants did not have current Mandated Reporter Training Certificates on file. This pose a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee agrees to submit to LPA proof of completion of the Mandated Reporter Training Certificates for S1, S2, and S3 on or before the Plan of Correction due date of 06/24/2022.
