Based on observation, the children's sink is not operational. Licensee stated they use wipes to keep the children clean. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states they will ensure the sink is operational, and provide of such to LPA by POC date.
Based on observation under the children's bathroom sink, the lock to the door below the sink was broken, and upon inspection inside the compartment, there was a lotion that had the wording of "keep out of reach of children." The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states they will put a working lock securing the door to prevent access by children by POC date.
Based on observation of 3 cribs, and Licensee's statement that C1 utilizes the cribs, LPA found blankets in all 3 cribs. The licensee did not comply with the section cited above. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the blankets at the conclusion of the inspection. Licensee states they will review the regulation and provide of understanding to LPA by POC date.
Based on LPA observation, the licensee did not comply with the section cited above when inspecting the backyard, LPA found multiple tools, power equipment, an uncovered barbeque grill, and a peace lily plant which is toxic to children. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states they will make the backyard safe for children's use and provide proof of such to LPA by POC date.
Based on record review, the licensee did not comply with the section cited above where C1, C2, and C3 did not have emergency contact information. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states they will complete records for C1, C2, and C3 and show proof of correction to LPA by POC date.
Based on record review, the licensee did not comply with the section cited above where C2, and C3 were found to be missing their LIC 9227 which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above where a bottle of Clorox bleach was found unsecured in a cabinet inside the restroom that the children utilize. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states they will ensure a lock is in place if storing dangerous cleaning chemicals within children's reach. Licensee moved the cleaning agent while LPA was at facility. POC clear.
Based on observation, the licensee did not comply with the section cited above where an infant (C1) was found sleeping with several blankets inside a crib which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed all blankets from cribs that infants utilize. POC clear.
Based on record review the licensee did not comply with the section cited above where S1 and S2 were found to not have current Mandated Reporter training. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states they will complete their Mandated Reporter training and submit to LPA by POC date.
Based on record review the licensee did not comply with the section cited above where S2 was found to not have a CPR certificate. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states S2 will complete their CPR training and submit the certificate to LPA by POC date.
Based on observation, the licensee did not comply with the section cited above in carbon monoxide was not present at time of inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee purchased a carbon monoxide at time of inspection. Licensee will send a video or factime LPA Flores via phone or email, a video of the carbon monoxide working by 03/01/24.
Based on observation, the licensee did not comply with the section cited above in the bathroom utilized by children has hazardous items under the sink, such as lysol cleaner and hair products. The medicine cabinet has medicines, toothpaste, and other hygiene products that could be hazardous to children in care. This /posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will place child safety latches on the cabinets, drawers and medicine cabinet and send photos to LPA Flores via email or phone by 03/01/24.
Based on record review, the licensee did not comply with the section cited above in licensee did not have a current CPR/First Aide certificate on file at time of inspection. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee's CPR/First Aide expired on 1/14/24. Licensee will obtain a current CPR/First Aide pediatric certificate and send a copy of certificate to LPA Flores via email by 3/29/24.
Based on record review, the licensee did not comply with the section cited above in that 3 (C1, C3, & C4) out of 6 children enrolled did not have proof of immunizations on file, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit to LPA via email copies of C1, C3, and C4's immunization record on or before the Plan of Correction date of 02/28/2023.
