Based on observation, the licensee did not comply with the section cited above in the unlocked bathroom vanity in the hall bathroom contained personal hygiene products and aerosols which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will fix or replace the broken lock on the bathroom vanity in the hall bathroom to ensure the contents of the vanity are inaccessible to children in care. Licensee will provide proof to LPA via email by submitting a picture of the vanity lock and cabinet on or before the POC due date.
Based on interview and record review, the licensee did not comply with the section cited above in two out of two staff did not have current mandated reporter certificates on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and assistant will complete mandated reporter training and provide copies of certificates to LPA via email on or before the POC due date.
