Based on interview & record review, the licensee did not comply with the section cited above in last drill conducted 11/30/22 per licensee and record review which posed a potential health, safety or personal rights risk to persons in care.

Resolution: licensee will conduct an emergency drill and send LPA Flores via email a copy of an updated drill log by 6/29/23

Based on interview & record review, the licensee did not comply with the section cited above, at time of inspection there was no sleep log for child #1. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee was unaware she had to keep a sleep log for children under 24 months of age. Licensee will create a sleep log and submit to LPA Flores via email by 6/29/23.

Based on observation, the licensee did not comply with the section cited above backyard had cobwebs on large playground obstacles, bikes & scooter and tricycle soft edges on handles were missing/broken. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she cleans every morning, the yard gets filled with cobwebs overnight. Licensee stated at time of inspection the children were outside this morning earlier but only rode their bikes and a scooter around the yard. Scooter and tricycle need to have rubber handles or covered to prevent injury to children in care. Licensee will clean yard of debris & cobwebs, fix the toys with sharp edges and send photo to LPA Flores via email by 6/29/23.
