Based on bservation, the licensee did not comply with the section cited above in outside play area has weeds, debris and spiderwebs around the playhouse at time of inspection. Day care bathroom had toothbrushes in the medicine cabinet accessible to children. which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will clean the front yard from debris, weeds and spiderwebs from the playhouse area from spiderwebs by 10/12/23. Licensee stated the children have not been outside to play today. Licensee will place a child safety lock on the medicine cabinet or store the toothbruses in another location making them inaccessible to children in care by 10/12/23. Licensee will send photos to LPA Flores via email
Based on record review, the licensee did not comply with the section cited above in child #1 did not have immunizations on file at time of inspection. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain immunizations for Child #1 and/or school enrollment form by 10/12/23 and send proof to LPA Flores via email by 10/12/23.
