Resolution: Licensee will clear items from play yards or have children who are able to transition to cots. Licensee will write a letter of understanding regarding Safe Sleep Regulations and sign and date. Licensee will send to LPA via email.

Based on observation, interview, and record review, LPA observed a previously off-limits garage area converted into a room with child care taking place without notifying the department prior, therefore the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide an updated facility sketch indicating changes and area on and off-limits. Licensee will also provide a plan on how each space will be used and the hours of use. Licensee will send to the LPA via email.

Resolution: Licensee will ensure that she and Staff #3 have a complete file with all required documentation and submit a copy of Staff #3's file to LPA by plan of correction date of July 3, 2023. LPA provided Licensee a copy of all required forms.

Resolution: Licensee will find or create a 15 minute infant sleep log and submit a copy of the log she will be using to document infant naps to LPA by plan of correction date of June 5, 2023. LPA reminded the Licensee she is to chart the infants naps until their second birthday. LPA provided Licensee a copy of PIN 20-24 CCP for a reminder.

Resolution: Licensee will ensure Staff #3 has required immunization's and submit proof to LPA Wilburn by plan of correction date of July 3, 2023
