Based on observation, the licensee did not comply with the section cited above where cleaners and soaps were readily available to children in the children's bathroom, and under the kitchen sink. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will ensure soap that should be kept out of reach of children is stored in a locked cabinet, and provide proof of such via photograph submitted to LPA by POC date. Additionally, cleaning agents located under the kitchen sink will be relocated and proof of such to be submitted to LPC by POC date.

Based on observation the licensee did not comply with the section cited above where LPA noted ammunition was stored with 2 firearms that were in a locked safe. Additionally, a box of ammunition was not stored in a locked container. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will purchase a seperate lock box for the ammunition, and store seperately from the firearms. Licensee states they will provide proof of the lock box via photograph to LPA once it arrives.

Based on observation, and interview with staff, the licensee did not comply with the section cited above where the Licensee was absent more than 20 percent of the day today. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will submit a plan for future absences that require them to be absent more than 20 percent of the day to LPA by POC date.

Based on LPA observation and interview, the licensee did not comply with the section cited above. LPA observed a walker outdoors in play area, easily accessible to children. Child care assistant reports the walker is used while training children to walk. Child care assistant removed the walker during inspection. Photo taken by LPA. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee assistant removed the walker from at the time of inspection and was informed the walker can not be used at the childcare facility.

Based on LPAs record review, the licensee did not comply with the section cited above in Five out of Five staff did not have CPR/FIrst Aide that meets Title 22 regulations at the time of inspeciton. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee's CPR/First Aide was expired at the time of inspection. Four out of four staff hade CPR/First Aide on file that does not comply with Title 22 regulations. Licensee will ensure that Pediatric First Aide/CPR Certificates are obtained for at least two staff that are present in the facility at all times. Licensee will submit proof of correction to LPA Flores via email by 10/18/24.

Resolution: Licensee and staff will complete Mandated reporter trainings and provide current mandated reporter certificates via email to LPA by 10/07/2024.

Resolution: Licensee will send photos of the yard to LPA with Feces removed, Gate on left side of yard, and cobwebs removed from all toys and playground area. Licensee will submit via email to LPA by 10/11/2024.

Resolution: Licensee will submit photos of the kitchen sink area with cleaning compounds, poisons, detergents and toxins removed by 10/07/2024 via email.
