Resolution: Licensee will send assistants Mandated Reporter training certificate to LPA Woods by close of business on 06/18/2025 by email or text message.
Based on observation, the licensee did not comply with the section cited above in which the backyard fence was missing a bunch of planks and was in disrepair which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will make repairs to the fence by 06/21/24 and send LPA Woods photos of the repair by fax, text, or email.
Based on observation, the licensee did not comply with the section cited above in which an infant was sleeping in a crib on top of a small blanket which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will remove the small blanket from the crib on 06/07/24.
Based on record review, the licensee did not comply with the section cited above in which C1 did not have any signed forms in his file for the LPA to review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have the parent of the child fill and sign all of the require forms and place in the childs file by 0614/2024 and email LPA Woods the completed forms.
