Resolution: Licensee will get the immunization records for C1 and C2 from their parents and send them to LPA by 07/02/2025 by email, fax, or text message.
Based on record review, the licensee did not comply with the section cited above in which she and her assistant did not have a current mandated reporter training certificate which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee and her assistant will complete mandated reporter training and provide the LPA with the certificate by 07/02/2025 by email, fax or text message.
