Based on observation and record review, the licensee did not comply with the section cited above in the licensee left her assistant alone with 5 infants and 1 preschool aged child which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee called and had 1 infant picked up from the day care to be back in ratio. Licensee will review the "How many children can attend a family child care" video on ccld.childcarevideos.org. After review of the video, Licensee will send LPA a signed and dated statement of understanding to LPA by email by end of day POC dated 10/22/2025.
Based on record review, the licensee did not comply with the section cited above in which her 1 assistant does not have proof of immunization for MMR and Tdap which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will email LPA her assistants proof of immunity for MMR and Tdap by POC dated 11/21/2025 by end of day.
