LPA met with Latrice Hearnes, Site Supervisor and informed the nature of the visit. LPA observed 16 Children in care with proper teacher/child ratios observed. There was a total of 6 staff. LPA Whitmore conducted interviews with (S1) and (S2) on 11/20/2024. On 11/20/2024 (S1) stated that she was outside breaking State Pre-school 1. What I noticed was (S2) coming from her classroom with another child. The other children were already on the yard. I saw her scan the yard. The next thing I know she was calling my name and coming from the gate area. At that point that’s when she yelled at me. (S1) the gate is open. I asked her if she had all her kids, and she responded I don’t see (C1). At that point I told everyone to take their kids to the classroom and do a headcount. So, (S2) went out to the same gate that was open and went to the parking lot and that’s where she says(C1) was. She ran towards her and caught her outside the parking lot gate. (S2) took her back into the classroom. At that point we went back to the classroom and took a count and I talked to both teachers. There were 4 children present that day. (S2) stated she was inside the classroom with another child that was throwing a tantrum, and the sub was outside with three children. After bringing the child to a calm state, child and I went outside with the 4th child. I told my sub she can go on her break. As, my sub is transitioning to her break. I did a headcount. I noticed that one was missing. I checked all her hidden spaces because she likes to hide. I checked the gate, and I noticed that the gate was unlocked. I yelled over to my Site Supervisor the gate is opened. I called my sub to come off her break quickly. Site Supervisor asked me if I had all my kids, and I told her no (C1) is missing. Once the sub is outside, I went out of the gate and started checking. After checking in between cars I found her at the entrance of the gate. I picked up the child and went back to my supervisor. S3) was in the classroom and then it was time to go outside. The children were lining up in front of the door. (S2) stated she will stay back with the other children. (S2) told (S3) that she can outside. (S2) has 5 children in total now. After 5 minutes in break (S2) comes in the classroom and tells(S3) to come outside. (S3) found out that (C1) had got outside because the gate was open. (S2) looked inside the school but was not there. (S2) went outside the school and found(C1) On 12/4/2024 Licensing Program Analyst (LPA) Doris Whitmore observed the Kitchen Staff for VOA delivering the meals. The time was 8:51a.m. LPA Whitmore observed the Kitchen staff entering through the same side gate that the child left out of. After the kitchen staff went through the gate. LPA Whitmore walked closer to the gate. There is a latch on the gate. As, LPA Whitmore walked closer to the gate. LPA Whitmore observed that the gate was not fully closed. From a distance it may look like the gate is closed but it is not. LPA Whitmore took a picture of the gate. LPA Whitmore waited until the kitchen staff came back through the gate. LPA Whitmore observed the kitchen staff come back through the gate. The kitchen staff closed the gate hard. LPA Whitmore walked to the gates to make sure it was locked, and it was. This is the same gate that (C1) went out of on 11/8/2024 to the parking lot. On the gate there is a sign that says KEEP THIS GATE CLOSED. Based on the information obtained throughout the course of the investigation, which included interviews with facility staff and 1 child, there was a lack of supervision due to the gate being left open. Also, the teacher did not see the child exit off the playground. Child exit out of the side gate that leads to the parking lot. Chi
