EBRAHIM FAMILY CHILD CARE

Based on LPA's observation, the licensee did not comply with the section cited above in that LPA observed blankets in the crib and two playpens during the inspection. This is not allowed and posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure blankets and no other loose objects are inside or on the side of the crib or playpen.

Based on LPA Wheatley's observation and record review, the licensee did not comply with the section cited above in that licensee showed a chart with the children's names which she falsified and wrote the month December 2025. This requlation is required and mandatory. This posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure she is documenting the children (age birth to 2 years) the napping time daily with correct dates and times starting 12/10/25. The records must be kept for 3 years after the child is disenrolled. The licensee will record the napping times every 15 minutes. The licensee will submit a plan of correction to the Department by 12/12/25 via email. .

Based on LPA's observation and record review, the licensee did not comply with the section cited above in that the Mandated Reporter Training Certificate is expired. The licensee's daughter sent her a copy on licensee's phone which showed it expired June 2025. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will complete the Mandated Reporter Training and send a copy via email to the Department by 12/16/25.

Based on observation, the licensee did not comply with the section cited above in that LPA observed 5 children upon arrival. Four of the five were infants. The maximum infants licensee can have without an assistant is three. This posed a potential health, safety or personal rights risk to persons in care. Although the licensee is licensed for a large license, the license must revert back to regulations for a small license when no assistant is on the premises.

Resolution: Licensee will ensure that when she does not have an assistant, she will maintain the ratios and capacity of a small license at all times. Licensee will began immediately and submit via email a plan of correction to the Department by 12/12/25.

Based on record review and licensee statement, the licensee did not comply with the section cited above in Fire/Emergency drills were not completed which posesa poten tial health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete Fire/Emergency drills by POC date and at least every six months after date. Licensee will email LPA a copy of completed dril by POC date.
