Based on record review staff #1 and staff #2 did not have current Mandate Reporter Training the censee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will start checking documents every few weeks. LPA provided LIC 311D in Spanish.
Based on record review child #2 & #3 were missing sleep log the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee placed the sleep logs in a board to use as a reminder to log checks.
Based on observation, the licensee did not comply with the section cited above in on eout of one infant present was observed sleeping bucked into car seat which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Provider removed the infant from the car seat and placed in play yard. Per licensee, they will review the Safe sleep regulations with parents and staff and provide a written plan to ensure the the facility will comply with Safe Sleep regulations including children not sleeping in car seat. The plan will be sent to LPA via email by POC due date

Resolution: Licensee removed the infant bouncer. Per licensee they will ensure the bouncer is not accessible to the children in care.
