Based on observation, interview and records review, licensee did no renew her pediatric or CPR training which poses a potential health, safety and/or personal rights risk to persons in care.
Resolution: Licensee states she will email LPA a copy of cpr and pediatric first aid training certificate by POC date.
Based on observation, the licensee did not comply with the section cited above, the functioning wall heater does not have a cover, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will install a cover on thewall heater. A picture of the correction will be sent via email to LPA by POC due date.
Based on record review, the licensee did not comply with the section cited above in 2 out of two persons reviewed, licensee and staff did not have the Mandated Reporter Certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will ensure that the Manbdated Reporter training is completed and the certificate is sent to LPA via email.
