Based on observation and record review, the licensee did not comply with the section cited above. Licensee had a play yard for the infant in care in an inaccessible room and stated the infant will usually nap in her arms. During a review of records, LPA also observed the Licensee also did not have an Individual Sleeping Plan LIC 9227 for infant in care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee placed the play yard in an accessible room and agrees to place the sleeping infant in as soon as possible. Licensee also agrees to have the infants authorized representative complete the LIC 9227 and will email it to LPA by 04/24/2025.
Based on record review, the licensee did not comply with the section cited above. Licensee's Mandated Reporter Training expired on 02/28/2025, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete the required training online at www.mandatedreporterca.com and email the certificate to LPA by 04/24/2025.
