Based on observation the licensee did not comply with the section cited above. In the back of the shed area that is accessible, LPA observed various debris: rakes, broken brick stepping stone pieces, splintered wood debris with rusted nails sticking out, rusted metal peices, and a cactus plant with splinters. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would remove the debris and make the cactus plant and tools inacessible. Proof will be submitted to the Department by POC date 3/30/2023.
