Resolution: LPA reviewed with Licensee how to complete the LIC 627 Consent for Emergency Medical Treatment. Licensee agrees to email LPA completed copies of the LIC 627 by 05/15/2025.

Resolution: Licensee will ensure all stafff have a criminal record clearance prior to employment. Licensee will submit a written acknowledging all staff need a criminal record clearance.
