Based on observation, the licensee did not comply with the section cited above. Licensee has not obtained a life ring that is approved by the United States Coast Guard and has not been keeping a log for her daily inspections of the drowning prevention features which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee ordered an life ring that meets the requirements during the inspection and stated she will send LPA Nolan pictures of the life ring in the pool yard by 11/05/2025. The Licensee also stated she will log her daily inspections effective 10/30/2025 and send a photo of the log to LPA by 11/05/2025.
Based on interview and record review, the licensee did not comply with the section cited above. Licensee's Mandated Reporter Training was completed on 10/16/2023 and was not renewed by 10/16/2025. Staff #1 (S1) has not completed the required traininig which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she and Staff #1 will complete the training and send LPA Nolan the certification by 11/05/2025.
Based on record review, licensee failed to maintain a current CPR/FIRST AIDE certificate, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will make an appointment to complete CPR/FIRST AIDE class and submit proof to LPA by POC due date.
