Based on observation and interview, the licensee did not comply with the section cited above, Infant play pens used for napping were missing fitted sheets, licensee states she did not have fitted sheet for the play pen, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states will provide proof of purchase of fitted sheets for mattresses and will email receipt and photos with the fitted sheets on the play pens by close of business, April 25, 2024.
Based on observation and interview, the licensee did not comply with the section cited above, licensee states detached garage room addition was done without city permits which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will either restore the garage to its original stage, or submitt the proof to obtain city approval for the detached garage room addition by close of business 04/25/24.
