Based on interview and record review, the licensee did not comply with the section cited above in the case of 2 out of 2 assistant files missing record of pertussis and measles immunizations, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide copies of pertussis and measles immunizations for the two assistants to LPA Gallo by the POC due date. Copies can be sent by email to matthew.gallo@dss.ca.gov or by text to 916-208-3734.
Based on observation, the licensee did not comply with the section cited above due to the presence of a loose blanket in the crib of a sleeping 1 year old infant, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the blanket from the crib with LPA present and stated their understanding that cribs must be free of loose objects when an infant is sleeping.
