Based on record review, the licensee did not comply with the section cited above in that LPAs observed C1-C3 did not have updated immunization records. Additionally, C4 and C5 did not have immunization records on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of updated immunizations for C1-C3 and provide a copy of immunization record for C4 and C5 to Community Care Licensing (CCL) on or before the Plan of Correction (POC) due date above.

Based on record review, the licensee did not comply with the section cited above in that LPAs observed that S1 and S2 did not have proof of Tuberculosis (TB) clearance which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide proof of Tuberculosis (TB) clearance for S1 and S2 and submit proof to Community Care Licensing (CCL) on or before the Plan of Correction (POC) due date above.

Based on record review, the licensee did not comply with the section cited above in that LPAs observed that C1 is missing the Family Child Care Home Notifications of Parent's Rights (LIC995A) form which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have authorized representative(s) of C1 complete the Family Child Care Home Notifications of Parent's Rights (LIC995A) form and submit proof to Community Care Licensing (CCL) on or before the Plan of Correction (POC) due date above.

Based on record review, the licensee did not comply with the section cited above in that S2 did not complete the Mandated Reporter Child Care Providers Training (AB1207) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S2 complete the entire Mandated Reporter Training including the "Child Care Providers" section and submit proof to Community Care Licensing (CCL) on or before Plan of Correction (POC) due date above.
