Based on observation, interview, record review, the licensee did not comply with the section cited above in that 4 out 4 Infant Childrens files did not contain a sleeping log readilyt availabe upon request which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will document a sleep log for each infant in care and email LPA a copy of each child's sleep log by the plan of correction due date of 04/11/2025.
Based on observation, interview, record review, the licensee did not comply with the section cited above in that 4 out 4 Infant Children's files did not contain an Individual Infant Sleeping Plan (LIC 9227) was not readily available for review which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will obtain the Individual Infant Sleeping Plan (LIC 9227) and email LPA a copy of the forms by the plan of correction due date 04/11/2025.
Based on record review and licensee statement, the licensee did not comply with the section cited above in 3 out of 3 Employees did not have Mandated Reporter Training on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and staff will complete Mandated Reporter Training and send Certificates to LPA by POC.
Based on record review the licensee did not comply with the section cited above in 3 out of 3 employees did not have their immunizations on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete staff files with required immunizations by POC.
