Based on observation, interview, record review, the licensee did not comply with the section cited above in that Marwin Serrano and Staff #1 did not have a current Mandated Reporter Training Certificate readily available upon request which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will complete the Mandated Reporter Training and email LPA a copy of the Mandated Reporter Training Certification for Licensee and Staff #1 by the plan of correction due date 05/24/2024.
Based on observation, interview, record review, the licensee did not comply with the section cited above in that Staff #1 did not have a current Pediatric CPR and First Aid certification readily available upon request which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated Staff #1 will complete Pediatric CPR and Frist Aid certification and email LPA a copy of the Pediatric CPR and First Aid certification by the plan of correction due date 06/03/2024.
