Based on observation, interview, and record review, the licensee did not comply with the section cited above in that the Immunization Record for Anna Badalyn, Licensee was not readily availbe for review upon request which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will get a copy of the Immunizaiton Record and will email LPA Calvillo a copy by the the plan of correction due date 05/10/2024.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in that Staff #1 did not complete LIC 9052 Employee Rights and was not readily available uopn request which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will have Staff #1 complete the LIC 9052 Employee Rights and will email LPA Calvillo a copy by the the plan of correction due date 05/10/2024.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in that Staff #1 Immunization Record was not readily available uopn request which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will complete the facility roster and email a copy to LPA Calvillo by the plan of correction due date 05/03/2024
