Resolution: Per Licensse, they will complete the children's records for current children enrolled and email a copy to LPA by the plan of correction due date 09/18/2025.
Based on observation, interview, record review, the licensee did not comply with the section cited above in Licensee, Co-Licensee, and Staff #1 did not have a current Mandated Reporter Training Certificate readily available for Department upon request which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will complete the Mandated Reporter Training and provide LPA with a copy of the completion certificate by the plan of correction due date 09/18/2025.
