ALGOSO FAMILY CHILD CARE

Based on observation, interview, record review, the licensee did not comply with the section cited above in that 3 of 3 children's files did not contain the LIC9227 Individual Sleep Plan readily available upon request which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated the will obtained the LIC 9227 Individual Infant Sleep Plan and email LPA a copy of the forms by the plan of correction due date 04/21/2025.

Based on observation, interview, record review, the licensee did not comply with the section cited above in that the play yard used by children has two (2) trampolines which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will remove the two (2) trampolines from the play yard and email LPA photos of the play yard by the plan of correction due date 04/21/2025.

Based on observation, interview, record review, the licensee did not comply with the section cited above in that 3 out 3 infant children's records did not contain a documentation of a physical check every 15 minutes while sleeping which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will physically check on an infant every 15 minutes while sleeping and keep a sleep log for each child and email LPA a copy of the 15 minute sleep log by the plan of correction due date 04/21/2025.

Based on observation, interview, record review, the licensee did not comply with the section cited above in that the Licensee and Staff #1 did not have proof of Immunization record and TB clearance or risk assessment readily available upon request which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will obtain proof of immunizations and tuberculsis clearance or risk assessment for Licensee and Staff #1 and email LPA a copy of the records by the plan of correction due date 04/21/2025.

Based on interview with licensee and record review, the licensee did not comply with the section cited above in licensee and staff #1 do not have mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and Staff #1 will complete training at mandatedreporterca.com, Licensee will send completed training certificate to LPA Lowe via email by end of business on or before 5/09/2022.

Based on interview with licensee and record review, the licensee did not comply with the section cited above in CPR/First Aid was expired and in need of preventive health and safety training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: 4/28/2022 Licensee called and scheduled CPR/First aid for end of May in front of LPA, Licensee stated will email cpr/first aid training and Prevenative Health and Safety practices to LPA by end of business on or before 5/31/2022.

Based on record review of staff and licensee files the licensee did not comply with the section cited above in immunization records were not in facility to verify staff is immunized which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send via email to LPA Lowe Staff #1 immunization record by end of business on 5/02/2022.

Based on record review of childrens records, the licensee did not comply with the section cited above in 3 out of 4 records reviewed were missing immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will request immunization records from families and email to LPA Lowe by end of business on or before 5/06/2022.
