Resolution: Licensee will submitt proof of complete PM 286 for six children to SJRO to the end of business day 06/22/2024.

Based on record review, the licensee did not comply with the section cited above in which assistant, Jada Freemen, has not completed Mandated Reporter Training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Assistant will take training for child care providers at WWW.MANDATEDREPORTERCA.COM and licensee will submit a copy of the certificate of completion to the San Jose Regional Office by close of business on December 22, 2022.
