LPA observed several containers of Clorox wipes in an unlocked cabinet as well as in another cabinet bug spray, lotion, perfume and a staff water bottle and soda (coke) container in the cabinet that had a broken door that was not on the cabinet in the sunshine class room, which poses/posed a potential health, safety or personal rights risk to persons in care.

interviews conducted and LPA observations that the facility had a issues with rats in the facility. It was also determined that the facility is cluttered and dirty and needs toys and equipment cleaned. This is an immediate risk to the health and safety of the child in care

Interviews conduted and staff file review determined that staff #4 is a volunteer & has been alone in the class room and on the play ground with more then 12 children and is not qualified. This poses a potential health and safety risks to the children in care.
