Resolution: Licensee stated , they will complete the training and email ccertificates to LPA by POC date.
Based on licensee's file review, the licensee did not comply with the section cited above and did not have current required mandated reporter training certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will complete the required mandated reporter training by 10/05/23 and a copy of certificate will be submitted to LPA via email.
