Based on observations and interview the licensee did not comply with the section cited above , Licensees fire extinguisher was serviced 10/27/2023, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will purchase new fire extinguisher and will submit proof of receipt to LPA by POC date .
Based on observations and records review , the licensee did not comply with the section cited above. LPA observed licensee`s cerificate expired 11/21/2024. Per licensee she did not have a time to complete the training , which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will complete the training and will email copy to LPA by POC date .
Based on licensee file review, the licensee did not comply with the section cited above and did not have current mandated reporter training certificate, licensee's mandated reporter training was expired on 01/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will complete the required mandated reporter training by plan of correction due date 12/08/2022 and a copy of certificate will be submitted to LPA Eivazian via email.
