Based on observations, records review and interview ,the licensee did not comply with the section cited above. # out of 12 were missing immunizations . Licensee was able to obtain 2 during the visit , she still missing one child`s immunization,which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain immunization from parent , will print and keep it in the children file and LPA will be notified.

Based on records review and interview , the licensee did not comply with the section cited above. 3 out of 12 were missing medical consent form, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she has forms ready she will obtain signatures from parents and notify LPA by POC date .

Based on observations , records review and interview ,the licensee did not comply with the section cited above. Licensee did not have a file for child #13 ,which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she understands the policy and she will obtain file and notify LPA by POC date .

Based on observations, records review and interview, the licensee did not comply with the section cited above.LPA observed only 5 signed receipts, seven were missing . Per licensee sha has only 5 parents sign , she still need 7 parents signatures, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she has forms ready she will obtain signatures as soon parents will come and will submit for LPA review by POC date .

Based on observations and interview the licensee did not comply with the section cited above. Licensing staff observed latch under teh sink was not working and cabinet was accessible to the children with Clorox spray inside, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will move cleaning supplies into the laundry area inaccessible to the childen .

Based on facility file review, the licensee did not comply with the section cited above and current facility roster was not available during this inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will submit her facility current roster by 12/04/24.

Based on LPA's observations on 12/03/24, the licensee did not comply with the section cited above during this inspection total of 16 children were present in the facility, all being under 5 year old which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During this inspection four children were picked up. Per licensee, she will disenroll four children effective today and an updated facility roster will be emailed to LPA Eivazian by 12/04/24.

Based on 16 children file review, the licensee did not comply with the section cited above six out of 16 children file was not available today for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will have 6 children file available for review by 01/03/25.

Based on staff file review, the licensee did not comply with the section cited above and licensee's assistant, Vardanush Avetisyan did not have proof of immunization against measles, pertussis and influenza on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, her assistant, Vardanush Avetisyan will have proof of immunization against pertussis, measles and influenza by 01/03/25.

Based on 16 children file review, the licensee did not comply with the section cited above and six children out of 16 did not have immunization record on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will obtain 6 children immunization record by plan of correction due date, 01/03/2025.

Based on staff file review and Licensing Information System, the licensee did not comply with the section cited above and her two present assistants during today 's inspection Anush Minassian and Zarine Melkomyan did not have criminal record clearance on file which poses an immediate health, safety or personal rights risk to persons in care. $1000 Civil Penalty was assessed during this inspection.

Resolution: Per licensee, Anush Minassian and Zarine Melkomyan will not be present in the home until they have criminal record clearance in Licensing Information System (LIS).

Based on facility roster review, the licensee did not comply with the section cited above and did not have current facility roster which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will have facility current roster by due date, 07/31/23 and a copy will be submitted to LPA via email.

Based on licensee file review, the licensee did not comply with the section cited above and did not have current mandated reporter training on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will complete required mandated reporter training by due date, 07/31/23 and a copy will be submitted to LPA via email.
