Based on records review, interview the licensee did not comply with the section cited above. Licensee did not have certificate for Pediatric first aid and cpr, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will complete the class and send proof to LPA by POC date.
Based on staff file reviews, the licensee did not comply with the section cited above and Diana Avagyan and Hasmik Ashughatoyan, licensee's assistants did not have proof of immunization against pertussis on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, her two assistants will have proof of immunization against pertussis by plan of correction due date, 05/31/24 and copies will be emailed to LPA.
Based on staff file reviews, the licensee did not comply with the section cited above and licensee's assistant, Hasmik Ashughatoyan did not have TB test clearance on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, her assistant, Hasmik Ashughatoyan will be TB test clearance on file and will submit a copy to LPA via email.
