Based on LPAs observation and director statement, the licensee did not comply with the section cited above and facility does not provide AM/PM snack to children and menus were not available to review during this inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observations , interview and records review the licensee did not comply with the section cited above. During the tour , LPA observed 8 cribs in the indoor activity area , which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observations , records review and interview , the licensee did not comply with the section cited above. During the file revew LPA observed 3 out of 5 were missing immunizations records, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observations and records review , interview , the licensee did not comply with the section cited above. Duirng the file review 2 out of 5 were missing Mandated Reporter Training certificates , which poses/posed a potential health, safety or personal rights risk to persons in care.

Reporting Requirements The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s). This requirement was not met as evidence by... Per staff#1, facility director has been suspended as of 03/20/24 and it was not reported to the licensing and facility did not have a designated teacher to act as child care director in director's absence. This poses a potential health, safety and personal right risk to the children in care.

Food Services (a) In child care centers (22) Equipment necessary for the storage,... of food or snacks ... shall be well-maintained. Necessary equipment shall include, but not be limited to: (C) Refrigeration. This requirements not met evidenced by: Per staff memebers , refrigerator does not keep cold temperature.LPA observed children`s food (milk, yougurts) stored in the refrigerator and freezer at degrees above 45F. Temperature in the freezer measured at 60.62 F(15.9C) and refrigerator at 61.8 F(16.6 C).

Child Care Center Directors Qualifications and Duties When the child care center director is absent from the center, arrangements shall be made for a fully qualified teacher as specified in Section 101216.1(c) etc... This requirement was not met as evidence by... Per staff#1, facility director, staff#3 has been suspended as of 03/20/24. Parents and staff were informed. Per staff#1, they did not have a designated teacher to act as director since director absence. This poses a potential health, safety and personal right risk to the children in care.

... Based on staff#1 and staff#4 interviews, on 07/20/23 from 7:30 AM to 8:30 AM , staff#4, teacher assistant supervised 2 infants alone. Per staff#3, on 07/10/23, she supervised 5 infants alone. This poses an immediate health, safety and personal right risk to the children in care.

... Based on LPA Eivazian observation, on 07/20/23 an infant was napping in the crib room from 9:45 AM to 10:10 AM with door closed, none of the staff check on the napping infant in person in the crib room. This poses a potential health, safety and personal right risk to the children in care.

LPA Eivazian sign in/out sheets from 11/01/21 to 12/20/21 and observed in different days there were about 13 to 25 children (infants and preschoolers) present in the facility between 7:30 a.m. and 8:30 a..m.. with one teacher and two teacher assistant.
