Based on observations , interview and records review the licensee did not comply with the section cited above. LPA observed 2 satff members were missing tb test and another one was missing mmr, pertussis, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observations, records review and interview ,the licensee did not comply with the section cited above. LPA observed only 2 satff memebrs , one staff memeber had 3 completed units, and other staff member did not have units, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observations and interview , the licensee did not comply with the section cited above. During the tour LPA did not observe shaded area. Per licensee , tent broke due to the high wind, and was not replaced , which poses/posed a potential health, safety or personal rights risk to persons in care.
