Based on observations , interview and records review , the licensee did not comply with the section cited above. Fire extinguisher in the home had a service tag with the date indicating 07/19/2022, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licesee stated she will call the company to service Fire extinguisher or purchase new one .
Based on records review aand interview ,the licensee did not comply with the section cited above. Two out of two assistants were missing proof of measles immunization, one out of two were missing pertusis immunization, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licnesee stated hae will obtain proof of immunization from assistants and will email proof to LPA by POC date .
