Based on facility file review, the licensee did not comply with the section cited above and per licensee she did not conducted fire drill every six months as it is required which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee she will conduct fire drill and document time and date on the log and a copy of fire drill log will be emailed to LPA by 09/30/25.
Based on licensee's file review, the licensee did not comply with the section cited above and licensee did not have required mandated reporter training certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will complete required mandated reporter training by 09/30/25 and a copy of certificate will be submitted to LPA.
