POGHOSYAN FAMILY CHILD CARE

Based on facility file review, the licensee did not comply with the section cited above and facility roster was not current which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will have her facility current roster available by 08/29/25 and a copy will be emailed to LPA.

Based on facility file review and licensee statement , the licensee did not comply with the section cited above and last drill was conducted 12/2024 and it was not conducted every 6 months as is required which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will conduct a fire drill, will document the time and the date and a copy of fire drill log will be emailed to LPA.

Based on children files review the licensee did not comply with the section cited above and per licensee she did not complete infant nap chart for current three enrolled infants which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, an adult supervise napping infants all the time. However, effective today she will complete infant nap chart every 15 minutes and a copy will be emailed to LPA by 08/29/25.

Based on seven children file review, the licensee did not comply with the section cited above in 2 out of 7 children reviewed files did not have immunization record on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will have immunization record for child#3 and child#4 on file by 08/29/25 and copies will be emailed to LPA by 08/29/25.

Based on six children file review, the licensee did not comply with the section cited above in 1 out of 6 children, child#6 did not have proof of immunization on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will obtained child#6 immunization record and a copy will be submitted to LPA Eivazian via email.

Based on LPAs observation, the licensee did not comply with the section cited above, upon LPAs arrival to the facility on 09/21/23 there were total of 8 children present in the home, 2 being infants with licensee's assistant only which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, effective 09/22/23 she will disenroll two children from her daycare and an updated facility roster will be submitted to LPA. A written declaration will be submitted to LPA by 09/21/23.

Based on children file review, the licensee did not comply with the section cited above and from 8 enrolled and present children only 5 children files were available for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will have all present children files available for review by plan of correction due date 09/29/23.

Based on facility file review , the licensee did not comply with the section cited above and licensee did not have current facility roster available during this inspection for review which poses/posed a potential health, safety or personal rights risk to persons in care.
