Based on Program Supervisor and LPA's observations Room 34 currently is used by School District Special Need Preschoolers and Room 35 is used by school age children, however Room 34 and Room 35 are licensed for this preschool program , the licensee did not comply with the section cited above and did not report classrooms changes to Licensing in a timely manner which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on LPA's observations and Program Supervisor confirmation, the licensee did not comply with the section cited above and LPA observed two big trees roots which are located on the outdoor playarea across from the administrative office are enough out of the ground which can be tripping hazard for children which poses/posed a potential health, safety or personal rights risk to persons in care.

(1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test.
(d) The surface of the outdoor activity space shall be maintained: (1) In a safe condition for the activities planned.

Based on 4 staff interviews, video footage, on 06/24/25, child#1 went outside the school and came back to lobby unsupervised, approximately for 151 footsteps far between 4:24:07 PM to 4:29:07. The facility was inspected and water samples were collected on 04/12/23. Sample Site D, two water faucets in Room 1107 were reported with 15.300 ppb or greater lead exceedance levels. This poses a potential health, safety and personal right risk to the children in care.
(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children.
